Flatonia Oak Manor Rehabilitation and Nursing Center is nestled in the small community of Flatonia , Texas just off I-10, halfway between Houston and San Antonio. We provide an activity program, on-site rehabilitation, respite care, and a secure dementia unit in Long Term Care.
Our pride and joy is definitely the Terrace. This 14 bed secured unit is designed for resident’s dealing with Alzheimer’s disease, related dementia and wandering. The Terrace has a specialized program that is very success oriented, activity driven and includes field trips of its own. It also has a beautiful secure courtyard accessible only to the unit.
Short-term Services -
If you are seeking short-term medical and/or rehabilitative services, we will focus our efforts working closely with you and your family on achieving the most successful functional outcomes so that you can return home as quickly as possible. Our team of healthcare professionals will develop an individualized care plan that is designed to address your specific needs. Supporting the nursing team is a group of licensed therapists who provide physical, occupational and speech therapies for patients recovering from major surgery, stroke, neurological and orthopedic conditions and other illnesses, injuries and disabilities. For those patients with a primary goal to return home, the focus will be on achieving the most successful outcome to be able to return home as quickly and safely as possible. A home assessment will even be done to ensure a return to a safe environment.
Long-term Care Services -
Our long-term care units are designed for individuals who need 24-hour nursing care and may be chronically ill, frail or experiencing a very slow recovery from an illness or injury. We offer private and semi-private room accommodations, beautiful common areas as well as outdoor patios ... In addition, we offer respite or vacation care that allows families an opportunity to take a brief break from the demands of caring for a loved one round-the-clock.
We accept Medicare, Medicaid, Private Pay, and many managed care plans accepted.
